There is the question about Former Wife Family Name. I know that I am supposed to put her maiden name. Would this be her name at birth or her name she had prior to marriage?

She had her first and last name changed about 5 years before we got married. The combination of her current first name and her birth name would never show up on any documents.

Timeline

Maiden name = A woman's family name before she is married. Used of a surname that is replaced by a woman when she marries. Also called birth name.

If she received a new birth certificate with her name change, then that's the name you use. Otherwise use the name in her birth certificate.

Keep in mind, this is for my former wife, not my fiancee.

Won't there be an issue since her first name is different than her birth first name?

Making up a name, let's say Jane Smith was her given name on her birth certificate.

When she was an adult, she changed her name to, say, Kelly Johnson.

When we were married, lets say her name became Kelly Jackson.

Should I really put her name like this?

First name: Kelly

Maiden name: Smith

She never would have gone by this name. And there would be no record of this person existing.

On the divorce decree, her name before and after would have been, in this example, Kelly Johnson.

Usually after a name change they issue a new birth certificate with the NEW name. Did she get a new birth certificate after her name change?

She did not. Back when I was in the Navy and we were still married, there were a couple times when I had to provide the legal name change document along with her birth certificate to prove her identity. These are both documents I no longer have access to.

Then use her name before you got married (the one after the name change)...

Strange that she didn't get a new birth certificate... A legal name change means you get a new birth certificate basically.

put the name she uses now it also ask for other names use so you can put any other name there that she had or still use
