Your GP should have a copy of your records. Sometimes they have not all been put on the computer so the office would have to dig out paper records. Call them and request immediately so they have time to retrieve them. Not all offices are keen to cooperate. If your mum has the little red vaccination book from when you were a child, the visa clinic accepts that as well.

They offer shots for a fee at the visa medical exam in case you are not able to get them free at the GP beforehand. You should only need two shots at the most.

Mumps/measles/rubella

Tetanus/diphtheria/pertussis (or Tetanus/diphtheria)

Waived--flu shot. It's not currently flu season

Waived--varicella. If you tell them you had chickenpox.

Waived--everything else on the list because they are not age appropriate for adults.

Ask if there are any records. Some practices give people their shots free on NHS, but they don't have to since it's for immigration. You don't really need to see the doctor if you can get an appointment with the nurse who does shots. It might be worth asking if you can get shots and if they charge.

You keep asking about shots at the embassy. The medical and shots are not given at the embassy. The doctor office is in London. They can give you the shots. You will go to London to Knightsbridge Doctors for the medical exam, then go back to London for the interview at the embassy.

Here is the website of the visa medical doctors where you should make an appointment a few weeks before your interview. http://www.visamedicals.info/us.asp Once NVC gives you an interview date, call for an appointment.
