Hi all,

Been reading through the fantastic K1 guide, and have a couple of questions which I'd really appreciate some advice on:

- Just to confirm something regarding the I-123 affidavit - my US fiancée doesn't have to file this over there, instead has to send me the signed form along with copies of her evidence documents for me to present at my interview?

- Secondly, what counts as a 'serious' condition on the health questionnaire. Does childhood asthma count as something I should tick 'yes' for? Also, guessing going to hospital as a kid for childhood fits also counts for the 'hospitalised' question?

- Thirdly, by a report from the doctor, should I ask them to make a written statement about my medical history noting the asthma (and me no longer suffering it) and my chickenpox contracted as an adult (helping me out with the pesky varicella jab we don't have over here). Did anyone ask the doctor to include anything specific or are they likely to include all this stuff? Guess it's worth scheduling an appointment to talk it through with them beforehand?

Any advice on those would be very helpful. Thanks in advance!

I-134 Affidavit of support is completed and signed by the USC and sent to the beneficiary to be taken to the interview.

Childhood asthma is not a serious condition. You do not need a letter, but if you feel better getting one, there's no harm in asking your GP.

Say yes to being hospitalized and you can explain at the medical appointment. They go through your answers on the form and you will have the opportunity to explain.

Not required

Not required. Knightsbridge take your word for it. No proof needed. Make sure before you leave that they check the box on the form that indicates viracella history. It's a pain to have to try get it sorted after you leave! And they can't give it to you at the medical because as you say, it is not available in the UK so if your history box is not checked, you would need to get it in the US to proceed with AOS.

No, in my opinion, totally unnecessary and a complete waste of time.

There is also a thread called Knightsbridge Medical (or something similar) which has slipped a few pages back, but covers personal accounts, hints and tips from those who have already attended. It's long, but worth a read, even if you only tackle the posts from the last 6 months or so.

I'm sure you'll be fine. You're asking your questions now, not the night before the medical so you'll be prepared :)

I'm keen to get the vaccination form complete to, hopefully, avoid having to go through the process when I get to he states ahead of AOS as I understand that can be a pain, and an expensive one at that.

It has some broken links now and some correction added toward the end so check the corrections. Phones now okay in embassy, 4 photos for medical, can't call DOS for info...to name a few.

It is form I-134. Yes he sends to you along with proof of income like an employer letter or 2014 tax return. You take to interview with your other documents.

- Secondly, what counts as a 'serious' condition on the health questionnaire. Does childhood asthma count as something I should tick 'yes' for? Also, guessing going to hospital as a kid for childhood fits also counts for the 'hospitalised' question?

Use your judgement. They don't have any way of seeing your medical records, so whatever you feel is important to mention on that medical questionnaire. Of course if you have a scar where you had your appendix removed, they will know so should be listed as a surgery. If you have high blood pressure they will know because they test that. Diabetes, or a heart murmur, or treated for cancer...yes those are important. Childhood fits? Do you mean seizures? epilepsy? High fever? Could be significant.

No to written verification of chickenpox. They take your word at the medical. Just be sure to tell them to put it on your form. Sometimes they don't bother to ask so tell them.

Doctor report. Ask him to write up a short report about when you were diagnosed. Did you have surgery, medication. What meds, how long....how it was treated. And do you continue to have problems with that condition. Do you still require medication? Or do you no longer have any health problems related to the condition. Yes people ask for specifics....."I need you to write up my diabetes....my depression" ....whatever you are reporting.

Yeah, I suspected auto-correct may have been the culprit there, but it was more for the benefit of those new to the process that I corrected it, rather than to point it out to you :)

Thank you, and yes, I plan on flying out as soon as possible. Just waiting for the visa to be issued. The long weekend didn't suit me this time round!

Getting the vaccinations done before the medical is the sensible thing to do. There have been many posts by folk who have endured the nuisance and expense of having to find a civil surgeon to get this done in the US. If you have time to search through the UK forum for past posts on the medicals at Knightsbridge Doctors you will find all kinds of things to look out for when and before you attend.

You're nearly there now, too. There are two 'Progress' threads running on this page at the moment. They are both for those who are reading up about and/or progressing through the stages between NVC post-approval and POE. It's quite nice that there are two. The first one started by Shana started earlier and covers the experiences, questions and answers by members who have (mostly) already received their visas now. There is a newer one started at the end of March for those approved more recently and none of us have got to POE stage yet. Both are valuable sources of information and support.

Great, thanks. The childhood fits were something I grew out of, and weren't epilepsy, so not sure exactly what to call them. Might be worth consulting my GP about them in that case.

I haven't had any medical conditions which have required treatment since the fits and the asthma, so not much for the doctor to write about... Although that in itself might help?

(Well, nothing except the chickenpox and back pain once, both a few years ago now haha!)

A further question - following the advice on the guide to get my fiancee to get her IRS transcript to help prove her income for the I-134 form, which type of transcript is it - Tax Return Transcript, Tax Account Transcript, Record of Account Transcript or Wage and Income transcript?

Also, when it comes to the documents for this (i.e. the tax transcript, letter from the bank, pay stubs etc.) how up to date do they and the form need to be. For example, we should be about a month away from getting our NOA2 at this stage - is it too early for her to prepare all this, sign the form and post it over to me, or should we wait until we receive the NOA 2 and then she can get the very latest pay stubs etc.

In other words, will stuff from x amount of months ago be invalid come interview at the embassy? Any ideas?

Tax RETURN transcript is a compact version of the tax return. That is what you need.

The tax account transcript is more like a ledger of your account...you owe this, you paid $x.xx on this date, you still owe $xx.xx and such as that.

London does not require everything you mentioned. They don't even require a tax return or transcript. Pick two good things at most. Employer letter is favorable to them. If that isn't easy, then tax transcript and maybe some pay stubs. Skip the bank letter.

You will have plenty of time to mail when you get notification of NOA2. There's still going to be a few months before your interview. But it also doesn't have to be hot off the press new either. My stuff was about four months old. The most recent pay stubs could even be scanned and emailed. They aren't that fussy really. If you do employer letter, it should be original as should the actual I-134 form.

Thank you - excellent stuff!

Yes, the employer letter might be tricky as my fiancee is a teacher and they've finished up for the summer already. Would her contract not be of use?

She can definitely do pay stubs, though, so might suggest she gets the tax return anyway in case of not being able to get the employer letter. That being said, she's back in August so at this rate there will be time for her to get a letter when she goes back after the summer!

The 'few months' wait for an interview doesn't fill me with hope... as I've intimated in another thread, I fear an October wedding might be overly ambitious now, what with the TSC backlog and all. It didn't seem unreasonable back in December!

Thanks for all your help so far - only been on the forum a couple of days and already picked up so much good advice. Look forward to sharing my own with others after the process has been and gone.

And actually just FYI: the IRS website was hacked. That means the online printable version of the transcript is currently locked down for an undetermined amount of time. You'll need to select the option to obtain it by mail - they only give you two choices return or account. Answer the simple questions and select it to be for the return, and they'll send it out in 5-10 working days. I would not bother with any of this until you get your NOA2.

Thanks. Yes, I'd noticed that on the IRS site. Was just trying to get ahead as suggested by the guide pre-NOA 2. Guess it's just good to be aware we need it.

Still, it helps make it seem closer by getting more stuff prepared. I'm going to get my vaccinations and police record sorted in the next few weeks just to feel like we're making some progress!

She can do the phone order thing where they mail it. It won't expire so no reason to wait on ordering the transcript (or your police certificate and the shots.)

If her teaching contract lists her salary, then it's like an employer letter. It's from the employer, says she has a teaching job, and tells her salary (hopefully). It doesn't have to be an actual letter.

Apologies for reviving the thread, but just had some questions regarding the vaccinations I'll need for for AOS - I want to get them done here and get the Knightsbridge Clinic to sign off on them to avoid problems and excessive charges in the States.

I'm 26, and looking at the CDC website I make it that I need the following:

  • Td/Tdap
  • MMR
  • Varicella/have had chickenpox
  • Flu

Firstly, is that correct based on your experience (for those who have done it)? Secondly, I'm a bit confused with the guidance on Td/Tdap. It says Td is needed for those 7 and older, with Tdap needed for 10-64 year olds. What's the difference?

Having obtained my vaccination record from my GP (which doesn't say I had the MMR, curiously, despite my mother suggesting I did so I might have to have that again I guess?) it appears I had what is described as 'Tetanus/low dose diphtheria' in December 2003. It also says 'exp 5/05' which I can only assume means that it expired in May 2005. Do I need to have a new vaccination or will this cover the Td/Tdap bit?

Any help or suggestions would be much appreciated!
