Can anyone confirm that there is only one type of birth certificate in the U.S?

Hi, there

I heard someone saying long version or short version of birth certificate and it made me wonder if my fiancee's one is the right one. But as far as I know there is only one type of birth certificate in the U.S. Is there anyone who can confirm that?

FYI, what she got reads from the top as following;

Ohio Department of Health

Vital statistics

Certificate of Live birth

And some biographic info of her and her parents such as name, date of birth, origin and so on

It's an one page document and she copied both front and back of it. Is it alright to submit this one as birth certificate?

Also, do I(beneficiary) need to submit her original birth certificate for an interview later on? I think I don't, right?

Thank you for your help.

Short forms include all the basic info, but don't include extra info about the parents. Long forms include all the basics plus the extra info about parents such as their full names, ages at the birth and where they were born too.

When I went to get our then-newborn son's birth certificate here in Texas, I learned that the long-form version is available in a child's county of birth. Had I gone to another county courthouse, they'd have been able to provide the short-form version only.

Other states might have similar or differing systems.

If the Ohio Department of Health provided the version that they gave her, that might be the only version available.

If you've copied the front and back, you've fulfilled the requirement in the I-129F instructions. I honestly can't remember whether the original is required later.

06-04-2007 = TSC stamps postal return-receipt for I-129f.

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

09-25-2007 = Touch (first-ever).

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

We got an RFE for providing the so-called short form instead of long form. Here's the deal: my fiance had birth certificate issued by the hospital, which by the way included all info about parents and places of birth and a lot more. USCIS asked us for a long form which is issued by the AUTHORITIES. Pretty much needs to be issued by the vital records office. When we ordered the long form and it arrived there was hardly any extra information compared to the hospital one, it was basicly a photo copy of a record from the vital records office on a fancy piece of paper. We sent it in and got approved after 4 days.

So you should be fine with providing what you have.

I believe you won't need birth certificate of USC anywhere else.

P.S: Fiance's birth certificate was from Oregon.
