Medical Documents- AOS from K1

Timeline

My fiancée had her medical in Chile this June and had all of her vaccinations completed. She has a paper that says "Vaccination Documentation Worksheet (for use with DS-2053 or DS-2054)". Is this sufficient to send with her AOS or are we missing something?

Yes. That's what we did.

It says on the I-693 (Medical Examination of Aliens Seeking Adjustment of Status), "you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form (sealing the original in an envelope and giving you a copy)."

Since my medical is still up to date, i can use my vaccination worksheet to include in my AOS. We submitted our AOS last October 6. We're still waiting for a response. Hopefully, no RFEs.

Just make sure you have completed your vaccination.

February 3, 2014 - I-129F NOA1

March 3, 2014 - NOA2!!! Our petition got approved!

March 17, 2014 - Packet Left from NVC to Manila

April 3, 2014 - Manila received our packet

April 30, 2014 - Packet 4 received

May 8-9, 2014 - Medical Exam

May 19, 2014 - Interview at the US Embassy; APPROVED!

May 22, 2014 - Status changed to ISSUED

May 28, 2014 - Visa on hand

August 8, 2014 - Entered U.S.

August 30, 2014 - GOT MARRIED!!!

October 5, 2014 - Filed for Adjustment Of Status

October 20, 2014 - Received Receipt Number via Email

October 27, 2014 - Biometrics Appointment Received

November 13, 2014 - Biometrics

March 24, 2015 - Approved

March 31, 2015 - Green Card Received
