Do we have 90 days to get married when she's handed the visa or when she arrives in the US?

Just curious, because we may want to remain in Bogota for a little more time after she is handed the visa at the consulate.

Does the clock start ticking as soon as walks out of the office or when she actually enters the US?

Also, if the 90 days begins after she flys in, is there a grace period between that time and when she actually receives the document at the consulate?

She has 6 months from her medical/visa interview (depends on the country) to fly to the US. The day she arrives in the US, the 90 day clock starts ticking.
