I looked at the example for the I-864, listed in http://www.visajourney.com/examples/INS-Form-I-864.pdf

And filled it and sent it out, my wife is immigrating from Belarus to USA, we got our NoA 1 and NoA 2 approval and case went to NVC, and took 3 weeks for them to input it into the Computer, and my wife place of appointment we choose Warsaw, Poland.

Now after paying for my Wife and Step Son AOS fees and Civil Documents Fee, I've sent 2 separate AOS packages one for my wife, and one for my Step Son, and after waiting week and half, I got an email for check list, I followed everything listed in that example above, this is what the Check list contained

Evidence Checklist)
Please review the instructions below then refer to the final page of this checklist for mailing
instructions.

Now I wonder what I need to really do to fix all these corrections, maybe I should say family household size is 2 instead of 3? as they both are coming together but they are separate cases

It sounds like you included your son in your wife's I-864. My observations in red. I hope this helps:

Form I-864, Affidavit of Support, & Financial

In Part 3, Information on the Immigrant(s) You are sponsoring, please correct the following...
[x] Items 2 through 6.f. This case's visa category does not allow accompanying family members; therefore, you cannot
list family members in this section. If you are a US citizen, then you have to mention only the principal beneficiary for whom you are submitting this I-864. You have to submit a separate 864 for your wife and one for your stepson.
[x] Item 7. The total number of immigrants listed in Item 7 must equal the number of immigrants listed in Part 3, Items
1 through 6.f. In addition, you must also verify that the total number of immigrants listed in Item 7 matches the
number you entered in Part 5 Item1. Since your now corrected I-864 will show only one sponsored immigrant, you will need to adjust this number in item 7.
[x]
In Part 4. Information on the Sponsor, please correct the following...
[x]
In Part 5. Sponsor's Household Size, please correct the following...
[x] Item 1. The number of sponsored persons must match the number listed in Part 3, Item 7. Again, you will need to adjust this section based on your correction for item 7.
[x]
In Part 6. Sponsor's Income and Employment, please correct the following...
[x] Items 13.a.1. The income you enter must match the total (gross) income reported on your most recent Federal
income tax return. If you filed Form 1040EZ, use the adjusted gross income instead. Check the tax forms or transcripts you are including with your I-864. If you filed IRS form 1040 or 1040A, then you need to mention the line that says "total income" in your tax form. If you filed IRS form 1040EZ, then you need to use the line that says "adjusted gross income."
[x]
In Part 8, Sponsor's Contract, please correct the following...
[x]
The Form I-864 we received was missing the following page(s). Please ensure you include all nine pages when you
resubmit the form. You have to include ALL pages of the I-864, even any pages you did not have to fill in anything, like page#8. This form is a contract between you and the US Government.
[x]
You must submit the following financial evidence: Not sure exactly what evidence you are missing. You would normally need to send 3 years of tax return, employment letter, and/or pay stubs for the past 6 months. Please make sure to read the I-864 instructions to check if anything else may apply in your case.

I did send them the employment letter, W2, and Transcript of 2011/2012/2013, Should I also send them the 1040?

Another thing, I did send them all 9 pages of I-864, and I don't understand if I have to fix everything or just the points with [x] and written words, Part 6 is what is confusing me at the moment as I filled up the Total Gross income, and I submitted W2 and pay stub and employment letter...

You will have to resend all 9 pages filled out again. Just correct the points marked x with the instructions beside them.

the problem here is part 6... I've checked my 1040 and transcript and its the same exact numbers I wrote for them in 13.a and 13.a.1 same with 2012 and 2011... not sure what is the problem... I checked both 1040 and transcript...

Sometimes the NVC makes mistakes on the income part.

The important thing to remember is if you had a 1040 or 1040EZ.

1040 = gross income

1040EZ = adjusted gross income

The three past years income should be from line 22 of the 1040 or if using a tax return transcript, it's the line labeled "total income".

If you filed your taxes using a 1040EZ, it will be the adjusted gross income on the return or the transcript.

Be sure you do not list any family members by name on the affidavits, as each has their own petition and affidavit. List your spouse as your spouse on the child's affidavit and just put the child in as a dependent on the spouse's affidavit. Make sure do not count your spouse twice on her affidavit.

Good advice from other members. In addition, you are saying that you submitted tax transcripts for previous 3 years plus W2's? Not sure if that is also part of your checklist but if you are sending transcripts, then you do not normally need to send W2's.
