Hi,

I just received my interview notice today and it has the checklist written on it. I was wondering about the medical examination (form I 693 and vaccinaion supplement in a sealed envelope.. it says here (unless already submitted) if I am not mistaken, the sealed envelope was given to the port of entry and when I submitted my AOS, i gave them the vaccination records, do I have to go get the I 693? I remember seeing here in VJ that the vaccination records would suffice.

Also, it is asking for the affidavit of support. (unless already submitted) I dont have a copy of the one I sent out so I was wondering do we have to prepare one still? Please give me advice. Thanks.

hello! if you are K1 visa and had your medical exam in st. luke's, you don't have to go through another medical exam in the US "as long as your Form I-485 (application to register permanent resident or adjust status) is filed within 1 year of your overseas medical exam . . . " (see link for instructions on I-693 http://www.uscis.gov/USCIS/New%20Structure...I-693instr.pdf).

i would suggest that you have a civil surgeon validate your DS3025 (vaccination documentation worksheet) from st. luke's prior to the interview. the doctor will go over the document and issue you part 2 medical examination of the I-693, which will be handed to you in a sealed envelope with the instruction "do not open: for uscis use only".

i went to the civil surgeon last monday and had my DS3025 validated by the civil surgeon. fee is 70usd. surprisingly, i was told that i need a vaccine for mmr. i already had 2 vaccines administered at st. luke's (and if i remember it right, one was for the mmr and the instruction to me at st. luke's was to have the tdap administered here as i was pressed for time). i also had a tdap administered at walgreens in august. anyways, i just told the doc to give me the mmr so i can be done with this requirement. it was 95usd. very confusing.

i also suggest that you have a copy of all the documents you have submitted with the I-485 during your interview.

p.s. i still have the cd of my x-ray. i asked the civil surgeon about it and he said to hand it over to the uscis when they ask for it.

You should get your vaccinations transferred from the DS 3025 to a I-693 by a designated civil surgeon. He will give you the I-693 in a sealed envelope for you to take to the interview.

Cost for civil surgeons vary BIGTIME!
Dr Arnold in Texas does the trip by mail for $30. LOTS of people here use him. Takes about 10 days.

I dont have a copy of the affidavit of support that we submitted the last time. I was wondering if I should make a new one?
